Good day

I have worked at a company through an agency for 3 months. My contract was terminated due to over-staffed reasons. I requested for my UI19 forms instead, i was called to work for 10 days without signing a new contract. I went to go work 4 days and decided to not continue to work unless the agency gives me a new contract. With failure of signing a new contact.I have not submitted a resignation letter because i did not sign a new contract. My question is, can i still receive my ui19 forms, how do i go about this matter.

I would greatly appreciate your expertise on this matter.
