Good day, 

This is a department of labour issue, they must be notified of this issue. 

They will send a labour investigation officer to your ex-employer and confirm if what you are saying is true. 

If it is true then they will request your UI19 form from the employer directly. 

You can always approach a court and bring an application to compel the employer to submit your UI19 form.
