Hi there, I was previously using recruitment agencies to seek for employment now one of the agencies is liquidated and I want to claim for my maternity benefits from the department however the liquidated agency gave me wrong/un-corresponding dates on my UI19  documents, I have no one to contact or email. The department won't help as the dates clash completely.

Please advise as I desperately need the money to pay off my debts that I incurred during my 4 months maternity leave.
